Excell-erate Weight Loss 12-Week Program

The following parts of the package are delivered by your Excell Rejuvenation provider:

• Initial 30-minute appointment

• Semaglutide prescription

• 15-minute follow-up at 12 weeks

• Portal access for medical questions related to semaglutide use 

• Options for continued prescribing with 6-month visits for an additional charge 

These are delivered by Jacqueline (Lady Leader Lifestyle)

• Health Lifestyle Clarity Consultation Call (20 minutes)

• A basic meal plan recommendation based on patient circumstances

• 1-3 most important action steps

• A PDF Starter Packet

• Includes both diet & exercise tips, templates, tools, & other exercises to help them with practical application.

• Email check-ins at week 3 & 9

• Patient/client will send tracking sheet details with any additional questions.

• Coach will respond with answers & feedback within 48 hours (usually much sooner). 

• Coach will send one check-in email if client doesn’t send initial check-in info at week 3, 6, & 9

• Custom macros

• Clarity Consult Call at week 6 halfway point at the end of week 6 and beginning of week 7. We make minor tweaks of personalized macros along with other exercise and lifestyle recommendations.

What does it cost?

This program is $1200 for 1 adult for the 12-week course. This program is not billable to insurance.
